Repeat Prescriptions

scriptYou can request your repeat medication by indicating the items you require on the tear-off slip of your prescription, and posting it to us or handing it in at the Practice Reception.  We also have spare forms at Reception for you to complete if you cannot find your tear-off slip.

If you enclose a stamped self addressed envelope we will post your prescription to you.  You can also fax a repeat prescription request to the Surgery.

Alternatively, you can order your repeat prescription via this website by simply following the link below and filling in the details appropriately.

It is Practice policy NOT to accept prescription requests over the ‘phone.

Medication Reviews: Patients on repeat medication will be asked to see either a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications.

Please allow two full working days between requesting and collecting your medication.
