Change of Online Prescription Service   If you use the online prescription service, you need to come into the Surgery to register for a new password - please bring along photo ID to do this. 

WE ARE A YELLOW FEVER CENTRE! You do not have to be registered with us to access this service. However, if you are not, you will be required to complete a form with basic information, before we can book an appointment.

Travel Clinic
We can provide a full Travel Health Service to ensure your health is protected when you go abroad. 

Out of Hours
We are covered by the Frimley Primary Care Services Limited, based at Frimley Park Hospital and accessed by dialling 111.

Test Results
We are extremely busy with telephone calls for the first 2 hours of the morning - to help us please do not phone for results until after 10.00 a.m.

Home Visits
Visits should be requested, where possible, before 10.00 a.m.  They are made only at the doctor's discretion.  Please ensure you leave a contact number and address when phoning, as the doctor is likely to phone you before making the home visit.

Surgeries have areas from which they register patients and you will need to check whether you live in our area before registration can take place. If you are not in our area we will advise you whom to contact.    If we agree to your registration you may complete the on-line form within pre-registration -  but please read the instructions - in red - carefully.  The form then needs to be brought to the Surgery to complete the registration procedure.  You will need to bring three forms of identification - one must be photographic ID such as a passport or driving licence.  The other two must be evidence of your address, in the form of utility bills which cannot be more than 3 months old. 

If you are registered with us and you move within Farnborough and remain in our area, you stay registered; if you move within Farnborough but outside our area you may request to stay with us - in writing. The decision will be made by the Partners, and you will be informed as soon as possible.  If you move out of Farnborough you will need to change to a doctor nearer to your new address. Your new neighbours may be able to help you and the local public library will have a list of GPs in the area.

stethoscope1Please note that when you register, your notes may be sent electronically from your previous doctor and likewise when you leave the Surgery they may be sent electronically to your new doctor.  Your personal information may be shared within the healthcare team and with other health providers.

If you have objections to either of the above, please write to the Practice Manager or Senior Partner.
